A workplace injury can affect daily life, relationships, identity, and emotional well-being. Therapy may offer support for that impact, but not every concern or service is necessarily covered by a claim.
Coverage and authorization depend on the individual claim and payer. This website does not collect referrals or reserve appointments.
Coverage and referral requirements vary by claim and type of service. Washington L&I State Fund claims are handled through L&I, while self-insured claims are handled through the self-insured employer or its third-party administrator. If you have questions about whether a referral or authorization is needed, contact Stephanie before scheduling.
For Behavioral Health Intervention (BHI) or mental health referrals, complete the printable L&I / BHI Mental Health Referral Form and submit it with appropriate supporting documents by fax or an approved secure/encrypted email method. Do not enter or send patient information through this website.

Each referral is reviewed for completeness, fit, and capacity before we coordinate next steps with the client.
